Job Description: Adult Day Service (ADS) Program Coordinator



In conjunction with the ADS Director, the Program Coordinator will implement the Life Circle Adult Day Services Program. This position will be responsible for designing and implementing an individualized activity program for 10 to 30 elderly adult day care program participants.

The nature of this work will be accomplished by:

● Well-developed plans of care

● Activity programing that meets the interests and needs of participants

● Facilitating nutrition that supports healthy aging

● Encouragement of healthy emotional expressions with other participants, staff and volunteers

● Cognitive stimulation that helps develops interest in living

● Physical strengthening through daily exercise

SALARY: $42,000 - $49,000

The ADS Program Coordinator shall meet all of the minimum qualifications and personal characteristics stated below:

1. Be a minimum age of 18 years of age

2. Shall have completed at least an associate’s degree from a nationally accredited institution of education in the field of geriatrics or a health related course of study which includes the care of elderly and/or handicapped adults. The equivalent in training and experience will be considered

3. Shall have a minimum of two years’ experience and training in services to elderly and/or handicapped adults

4. At least 1 year supervisory experience

5. Experience designing and/or implementing creative activities or educational programs for adults is preferred


In conjunction with the Adult Day Services Director and staff, the Program Coordinator will:

Complete a needs assessment during the initial client intake interview

Develop and maintain individual care plans

Design and implement activity programs that meet the specific needs and interests of the participants

Search for appropriate sub-contractors to provide unique activity programs and experiences for clients (e.g. music, art, cooking, yoga, etc.)

Meet weekly with the Director to review activity program and participant progress

Meet monthly with Board of Directors’ Volunteer Coordinator to review, coordinate and develop volunteer involvement in weekly and monthly activities

Hire, train, and supervise Caregivers to facilitate the smooth running of all day-to-day operations (personal care, recreational activities, etc.)

Ensure that caregiving staff and volunteers adhere to Life Circle policies and procedures and state licensing requirements

Help facilitate staff entries into participants’ files to assure that all events and incidences are accurately recorded

Work with staff to provide written quarterly participant progress reports

Participate in in- person six-month reassessments and reviews with families

Help facilitate participants’ daily nutritious balanced lunch & snacks

Other responsibilities as assigned by the Director

Serve as substitute Director in the absence of the ADS Director
